Position Summary: LifeMoves is currently seeking a family-centered Bilingual Community Case Manager to join our Upstream program, which serves and collaborates with referred families and youth in Redwood City, CA through community case management. Responsibilities include providing outreach in a school and community setting; intensive case management to households who are referred by Kennedy Middle School (in RWC) and are at-risk of or experiencing homelessness; completing screening assessments and case plans; making appropriate referrals; and collaborating with the team to continue the development and implementation of the Upstream program. The successful candidates will have the ability to represent LifeMoves in a diplomatic and professional manner with partners, funders, and other city/county entities and must be bilingual in Spanish to serve our primary Spanish speaking families.

Position Duties:

  • Serve as an initial point of contact for families wanting to enroll in the program
  • Provide a safe and welcoming space when interacting with families and youth
  • Conduct intake and assessment of enrolled households and make appropriate referrals to community resources/services
  • Collaborates alongside program participants to develop, implement, and revise respective case plans and consistently monitors progress toward participant goals
  • Communicate appropriately as needed with school district personnel on existing referrals
  • Maintains program participant and program records in accordance with applicable standards and regulations, grant requirements, etc.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
  • Participates in continuing education activities, remaining knowledgeable in areas of youth and family homelessness and existing community barriers
  • Facilitate or support workshops, parent training, and organizing cycle for families along their interest and professional development goals.
  • Provide support to parents to become active advocates within their school system
  • Contribute to the creation of a wraparound service model for the Upstream program at Kennedy Middle School
  • Responsible for delivery of services in an ethical, professional, and effective manner
  • Support Program Director in the evaluation of program procedures, policies, and metrics
Position Qualifications

  • Bachelor's degree in education, social work, psychology or related field, or a minimum of two years' case management/community work experience
  • Experience collaborating with families and communities impacted by unjust systems
  • Understanding of the issues faced by individuals experiencing homelessness and other marginalized populations struggling against systemic inequities
  • Experience working with migrant youth and families highly desirable
  • Experience working in a school system highly desirable
  • Experience working with individuals who could benefit from behavioral health resources
  • Excellent communication and interpersonal skills; proven ability to collaborate with community partners
  • An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
  • Multicultural humility desired
  • Bilingual Spanish speaking required
  • Availability to work occasional evenings and weekends
