Description:

POSITION SUMMARY: Under the supervision of the Social Services Manager, provides clinical direction in Recuperative Care Unit at both City of Refuge and The Gateway Center for guest as of the programs. RN will oversee hospital and community partner referrals for placement, medication education, general health check-in and follow-up with program guests. *Provides patient care and/ or support activities appropriate to ages served; primarily adults ages 18-65 or geriatric patients ages over 65. May also include care for infant age 0-1 year, child ages 1-12 years or adolescent ages 13-17 years. Ability to work utilize Microsoft Office, manage reports and work within a teamwork environment.
EDUCATION REQUIREMENTS: Associate's Degree or Bachelor's Degree in Nursing. CPR/BLS certified.
EXPERIENCE REQUIREMENTS: At least two years related experience preferably in an outpatient care setting or outpatient education program. Desire to work with underserved and/or disadvantaged populations.
JOB KNOWLEDGE: Ability to effectively assess and evaluate patients, to make and implement plans. Knowledge of adult and adolescent healthcare, patient education, diabetes management principles, and home care continuum. Knowledge of growth and development including the adult and children, ability to adapt teaching to the age of the learner, including, age 18-65, geriatric over age 65, low literacy populations, and patients with special care needs. Excellent communication skills to develop and teach programs.
