A large provider for homelessness is looking to add Case Managers to their support services based team!

Schedule: Monday - Friday - 40 hours per week. 8:30a-4:30p or 9a-5p

Job Summary:

As part of the Support Services Team, work with all aspects of the program, including outreach, community referrals, Rapid Resolution, intake and assessment, case management, rapid housing and housing stabilization. Develop relationships with landlords and property managers for permanent housing placement and retention. Liaison to all team members, collaborating organizations, vendors and other human services and government agencies. Work to ensure successful outcomes and program objectives for Veteran families who are homeless or at risk of becoming homeless. Dedicated to working with all three components of the Program - Rapid Resolution, rapid housing, re-housing within 90 days of losing housing, and housing stabilization and prevention.

Required Qualifications:

• Bachelor's degree in Human Services or related field or 3 + years community case management experience.
• Master's degree preferred.
• Experience working with a variety of behavioral health challenges, and/ or homeless population.
• Must understand and have ability to work with Harm Reduction, Motivational Interviewing and Trauma Informed Care best practices.

Pay Rate: Varies based on experience
