Description
Under general supervision, provides routine community health nursing care services to individuals and families and applies the procedures and standards of community health nursing at the local level within an established program; performs related work as assigned.
Examples of Work

Applies physical assessment techniques to determine treatment needs, and/or progress of the individuals and to assess the need for referral to other health care agencies.

Administers medical treatments, immunizations, and other diagnostic and therapeutic procedures under medical direction to assist other health practitioners in providing proper medical/nursing care services.

Refers individuals and families to other health care providers as needed to facilitate their receiving needed diagnosis and/or treatment.

Counsels and advises individuals, families, employers, civic group members, and others on health care, health care programs, and health problems such as disease and disability to promote and encourage use of nursing care services available in the community and to foster an attitude of self-help and personal responsibility for health care among individuals in the community.

Arranges and conducts screening programs such as diabetes, cancer and blood pressure screenings, and other health promotion programs to ensure health care services are available and accessible to the community.

Identifies and evaluates community health care needs to facilitate the assessment of community based health/nursing care services and to assist in the development or modification of health care and/or disease prevention plans.

Assists in planning and implementing community health care and/or prevention plans to ensure the necessary services are available and accessible to individuals and families and to facilitate the attainment of State health care plan goals.

Instructs and advises health care staff and/or student nurses within school, local health agencies, or home health care projects at the community level to ensure dissemination and learning of the principles and practices of proper community health care.

Records and summarizes health care information to ensure the availability of required statistical data and to facilitate the preparation of State and/or federal reports on health care/nursing care services provided.

Plans and arranges in-service education seminars and workshops to assist other agency staff through provision of content input on training and to facilitate the scheduling of speakers and the locating and publicizing of the training sessions.

Knowledge, Skills and Abilities

Knowledge of: the principles and practices of community health nursing care; the principles and practices of disease prevention within community based programs; the practices of health education; the social characteristics and economic conditions which affect the health of individuals and families; the cultural and environmental characteristics and interpersonal relationships of families and groups; the techniques of interviewing and counseling.

Ability to: communicate individually and in groups with medical practitioners, health care staff, other governmental officials, and the public; establish and maintain working relationships; understand and apply supervisory and medical instructions; record and retain health data; interpret and use health care information; make and implement health care decisions based on nursing care principles and practices.

Qualifications / Requirements
Post high school coursework/training in: community/public health nursing care; OR experience providing community/public health nursing care services; medical systems; or case management.

Licensed as a Registered Professional Nurse in the State of Nebraska.
