Description:
St. Mary's Hospital a member of Trinity Health Of New England is looking for a Primary Therapist /Social Worker to join our team of mission-driven healthcare professionals located on our Emergency Room and Behavioral Health Unit. The Social Worker / Primary Therapist provides psycho-social services to adult, child and adolescent patients seen in the Emergency Room and the Inpatient Behavioral Health Unit at St. Mary's Hospital. The Social Worker serves as the driver of a collaborative, interdisciplinary process, which coordinates, monitors, and plans behavioral health care services to meet the individual needs of our patients. The Social Worker/Primary Therapists works independently and interchangeably on both the Inpatient Acute Care Behavioral Health Unit and Emergency Department and will complete a comprehensive psychological and substance dependence assessments and will facilitate psychosocial groups to assist in providing strategies in managing psychosocial problems interfering with compliance with other treatment regimens. In addition, will incorporates case management functions into the daily activities by acting as a resource and guide for physicians, the entire care delivery team, patients, and their families.

Work Hours/Shift:
  • Per Diem - 12 hour shifts

You Will Be Responsible For:
  • Exceptional Patient Care: Contributes to overall quality of care through meeting the individual needs of our patients in order to improve the quality of care, reduce costs of care, and enhance patient satisfaction.
  • Excellent Communicator: Communicate effectively utilizing therapeutic communication with patient population. Excellent communication skills (written and verbal) with nursing staff, colleagues and physicians.
  • Technical Familiarity: Computer-based applications (email, and standard applications such as Microsoft Office).
  • Active Teamwork: Demonstrates problem solving, conflict resolution, and positive communication through teamwork

Requirements:
  • Education: MSW degree from an accredited School of Social Work or Behavioral Health Field required.
  • CT licensure in appropriate professional designation preferred, i.e. LMSW, Clinical Social Worker, Psychologist, Marriage and Family Therapist, or Professional Counselor.
  • Excellent written and verbal communication, knowledge of community resources, social work theory and practice.
