The Community Mental Health Center (CMHC) is a SAMHSA grant funded program. Its purpose is to enable the Community Behavioral Programs and Residential Programs in the Bronx support and restore the delivery of clinical services that were impacted by the COVID-19 pandemic. The program effectively meets the needs of children/youth with serious emotional disturbance (SED), adults with serious mental illness (SMI) and individuals with co-occurring substance use disorders.
The Bridges to Care team works directly with individuals with severe mental illness, substance use disorders, and/or physical health conditions. Working primarily in the community, the clinician uses a recovery-oriented lens and evidenced based approaches to evaluate participant's strengths and resources, identify barriers to engagement in services, ascertain level of risk and appropriate services to manage risk, assist with managing and keeping appointments, provide psychoeducation and coordinate other aspects of participants' health and community services to support engagement. The clinician also provides crisis services to participants remotely or in person in the office or community.
