Job Title: RN - Case Management

Duties & Responsibilities: These managers oversee the care of patients in their own residences, manage direct care providers, help with treatment planning, and support all involved in the operation.

Minimum Requirements:

• NY RN license
• Certified Home Health Care experience required
• Epic or Netsmart Required
• BLS
