Responsibilities

RN Care Coordinator

This role functions as the Registered Nurse (RN) member of the healthcare team in a manner consistent with their scope of practice in Pennsylvania. The RN is responsible for assessment, planning, implementation, evaluation and documentation of care provided and patient response to interventions.

The RN is responsible for coordinating care and linking children with special health care needs and their families to services/resources in a coordinated effort to maximize the potential of the children and provide them with optimal health care. Care coordination may involve but is not limited to the following: planning treatment strategies; coordinating visits with sub specialists; organize care to avoid duplication of diagnostic services; sharing information among health care professionals, other program personnel and family; facilitating access to services; training of caregivers, education of local emergency medical service when a child with special needs lives in the community, ongoing reassessment and refinement of the care plan.

Qualifications

Skills & Abilities:

  • Strong Clinical Skills
  • Strong Case Management Skills, extensive knowledge of insurance related issues
  • Ability to work independently, persuasively and collaboratively with all members of the inpatient/ outpatient health care team and family
  • Must be organized and have the ability to multi-task
  • Must be self-directed
  • Effective interpersonal and written communication skills
  • Must be able to demonstrate the knowledge and skills necessary to develop creative care coordination strategies based on challenging physical, psychological, developmental, social and educational situations appropriate to the age of the patients served

Training & Education :
  • Graduate of an accredited School of Nursing.
  • BSN required

Experience:
  • Strong Clinical background
  • Minimum of 5 years combined experience in pediatric acute and chronic care (e.g. Homecare, long - term care) setting
  • Minimum of 5 years pediatric case management and/or care coordination experience with children with special health care needs
  • Home care experience and/or pediatric office desirable
  • Pediatric nurse certification a plus

Required Licenses:

Current Pennsylvania Registered nurse licensure.
