Description:
Saint Francis Hospital, a member of Trinity Health Mid-Atlantic, is looking for an experienced Program Coordinator to join the team for our Tiny Steps program!

Shift
Schedule: Full-time day shift (40 hours/week)

Summary:
The Tiny Steps Program Coordinator and Community Benefit Support person provides direct programmatic support, data analysis, and community outreach for Tiny Steps and other Community Benefit Programs.
Responsibilities include, but not limited to:

• Collect and analyze Tiny Steps patient data in support of state grants; prepare material for annual Tobacco Grant Audit and monthly reporting functions
• Build and promote community awareness and outreach programs to enhance Tiny Steps patient education
• Monitors spend of grant dollars to ensure Tiny Step purchases are within budget
• Reconcile P-card transactions and receipts
• Track new patients and update labor and delivery files for Tiny Steps program reporting
• Serve as principal liaison to Bayard House, a licensed residential program providing direct client care services to at-risk, homeless, pregnant or newly parenting adolescents, young women, and their babies
• Assist in establishing office flow processes to ensure program delivery is effective and efficient
• Lead the implementation and communication of Community Benefit patient programs and activities in Family Practice office workflow
• Conduct patient interviews to determine eligibility for the Healthy Women, Healthy Babies grant
• Attend quarterly Healthy Women, Healthy Babies meetings and update Medical Director and staff on any changes; attend other community meetings as needed
• Manage internal Community Benefit reporting process, including event entry in the CBISA reporting system
• Represent Saint Francis Healthcare on monthly Community Benefit and CBISA teleconference calls
• Coordinate community benefit programs and activities with partner agencies
• Provide liaison support between internal community benefit programs and community resources for patients
• Support Community Health and Well-Being Social Influencer of Health programs and activities
• Assist Community Benefit Manager with Saint Francis Healthcare's Community Health Needs Assessment, and track progress on Community Health Improvement Plan

Minimum Qualifications:

• Associates Degree preferred or equivalent work experience required
• Bilingual preferred
• Clinical experience preferred
• Medical Assistant Certification a plus
• Strong Interpersonal Skills
• Excellent written and oral communications skills
• Strong Organizational Skills
• Proficient in Microsoft Office
• Willing to work some evenings and Saturdays
• Limited Travel
