Program Coordinator - Supported Living

The Supported Living program is seeking qualified and caring people to work as a Program Coordinator. Successful applicants will attend paid trainings.

RESPONSIBILITIES:

  • Ensure quality, health and safety and client centered approaches for individuals in community living services
  • Supervise, support, and train staff as they assist individuals served.
  • Ensure compliance with federal, state, and local regulations while expanding opportunities for persons served to participate in community life as they choose.
  • Develop and implement the Personal Centered Individual Service Plan
  • Influence and procure Plan of Care that addresses all client needs
  • Coordinate the delivery of all services rendered by Benchmark and other providers.
  • Assist people served in expanding their opportunities to participate in community life.
  • Provide on-call direct support and emergency coverage.
  • Participate in the hiring, training and supervision of Managers, Supervisors and Direct Support Professionals.
  • Accurate and timely completion of PCISPs, client financials, CQAs, monthly and daily documentation
  • Submit reportable incidents
  • Monitor client finances.
  • Serve as liaison with external and internal programs and family members.
  • Maintain employment required documentation and training as required to comply with federal, state, and program requirements and standards.
  • Comply with all standards to assure the health and safety of all individuals.

Q UALIFICATIONS :
  • Bachelor's degree in human services, preferred, but not required
  • One (1) year experience working with developmentally disabled people or five (5) years experience
  • Valid driver's license
  • Access to vehicle in good operating order
