Job Summary:Primary Care Facility seeks a Physician to work as a key member of a highly accomplished medical team. The physician will have extensive training in internal or family medicine with a specialty in the diagnosis and treatment of infectious diseases to provide the following services at Health and Wellness Clinic:
•Provide medical guidance and act as the subject matter expert, particularly on infectious disease or epidemiology-based events such an epidemic, outbreak, cluster or pandemic.
•Provide medical diagnosis and post-exposure measures such as antibiotics, etc.
•Maintain working relationships with other medical providers and/or agencies engaged in assessment and disease control efforts.
•Collaborate with medical case managers as a team member of Southern NH HIV/AIDS Task Force.
Basic Qualifications:•Doctor of Medicine degree - previous experience treating infectious disease preferred
•Passed the United States Medical Licensing Exam.
•Current registration and licensure in the State of New Hampshire or ability to obtain a license in New Hampshire.
•Minimum of 3 years' experience in Inpatient or Outpatient .
•Successfully complete all required company in-services.
Preferred Qualifications:•Knowledge of The Federally Qualified Health Center (FQHC) Regulations is a plus.
•Multilingual is a plus (Spanish preferred but not required).
•Knowledge of computers and software applications or ability to learn quickly.
•Experience working with people from diverse populations.
•Ability to work as an effective team member is essential.
Knowledge, Skills and Abilities:•Be aware of and adhere to HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency.

•Ability to make meaningful observations and document these findings.
•Ability to organize and prioritize work within caseload to meet patient/family/payer needs and required documentation within required time lines.
Essential Duties and Responsibilities:•Will treat patients from diverse populations.
•Review patient files sent by other medical professionals.
•Review medical history of the patients along with various lab reports and X-rays.
•Regular case consultations with case managers, primary care providers, others as needed.
•May request samples of blood, tissue or other bodily fluids from the patients for testing.
•Identify disease and prescribe treatment plans and medication for the patients.
•May have a case load of 10 - 20 patients per day. Follow up appointments as needed.
•Monitors the occurrence of reportable and emerging diseases, analyzes infectious and chronic disease trends, and report findings to the clinic director for further evaluation of the Community.
•May attend case review meetings, conferences and referral meetings.
•Other duties may be assigned by the medical director or clinic director.
