To help us achieve this mission, we are looking for an experienced physician who will apply PACE fundamentals to our innovative care approach, and lead the PACE clinical program, offering the opportunity to work autonomously but with the support from expert PACE operators, geriatricians and project managers.
Essential Responsibilities:

  • Partners with the PacificSource PACE Executive Director to establish PACE program vision, goals, and strategy.
  • Serves as the business and clinical lead for PacificSource PACE Medical group.
  • Leads the design/development of the PACE program's model of care, quality and safety programs, assessment tools, clinical practice guidelines, policies and procedures, risk stratification processes, and the effective implementation of the interdisciplinary team and care planning process.
  • In collaboration with the PacificSource PACE program's Executive Director, leads the development of an operating model that optimizes the balance between insourced capabilities, vendored capabilities, and capabilities purchased through a shared services model with PacificSource.
  • In collaboration with the PacificSource PACE program Executive Director, monitor and educate the IDT on total cost of care, cost-effective prescribing habits, medical appropriateness, and utilization reports and management. Participate in utilization and benefit determination reviews as needed.
  • Ensures adequate clinical staffing levels to meet program implementation, growth and development needs and all regulatory and contractual requirements.
  • Recruits, develops, and manages a team of high performing clinicians to ensure the provision of high quality, cost-effective care for all PACE program participants as well as other populations as appropriate.
  • Serves as the program's liaison to the local, regional, and national medical and PACE communities, building relationships to ensure program participants receive optimal care.
  • Builds and maintains strong relationships with physician groups, specialists, contracted providers, hospitals, and skilled nursing/post-acute rehab facilities to strengthen to ensure PacificSource PACE program participants obtain timely access to high performing care and achieve optimal outcomes. As needed, proactively address any issues related to the quality of contracted medical services.
  • Supervises all medical care and services delivered through the program and is accountable for achievement of program outcomes, goals, clinical performance, including quality, accurate and complete documentation, compliance, safety, and other indicators as appropriate.
  • Serve on appropriate organizational and corporate committees.
  • Ensures compliance with PACE provider regulations as they pertain to medical/clinical components of care.
  • Serves as a physician liaison to local, state and federal agencies, and the PacificSource Board of Directors.
  • Ensures systems and culture support continuous quality improvement in the delivery of care, outcomes and participant satisfaction.
  • Participates in national quality improvement efforts such as those organized by the National PACE Association, AMDA, and other professional groups.
  • Develops and oversees peer review systems for clinical staff and coordinates the performance appraisal of the primary care physician. In partnership with the Executive Director, develop and maintain clinic staff job descriptions.
  • Provides staff in-service training as well as community education and presentations in coordination with marketing as needed.
  • Makes staffing recommendations for PACE clinical staff.

Supporting Responsibilities:
  • Meet department and company performance and attendance expectations.
  • Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: At least 10 years of relevant experience, including a minimum of 5 years in a clinical practice.

Education, Certificates, Licenses: Doctor of Medicine degree required. Active and unrestricted Oregon Medical License, current board certification in a primary care specialty.

Knowledge: Knowledge of the principles and practice of geriatric care, hospice and palliative medicine, with emphasis on symptom control associated with chronic and terminal illness. Demonstrates understanding and acceptance of geriatric care principles. Ability to educate physicians, hospice staff and the community on hospice, palliative care, and end-of-life issues. Ability to work collaboratively on committees and as part of the interdisciplinary team with patients, families, primary physicians, referral sources, hospice staff and volunteers. Skill in organizing and prioritizing workloads to meet deadlines. Ability to effectively supervise and motivate other physicians and nurse practitioners. Ability to communicate effectively both orally and in writing. Ability to communicate effectively with co-workers and other customers.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 25% of the time.
