Licensed Practical Nurse (LPN)

Volunteers of America National Services (VOANS) is seeking a full-time Licensed Practical Nurse (LPN) to join their team in NC! Under the direction and supervision of the Clinic Supervisor, uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care and contributes to the evaluation of individualized interventions related to the plan of care. The Licensed Practical Nurse administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regards to plan of care, nursing treatments or medications. Communicates participant's condition to primary care givers, physician and interdisciplinary team.

The Licensed Practical Nurse will report to the Clinic Supervisor.

The Licensed Practical Nurse will work on-site in the Senior Community Care of North Carolina facility.

Essential Functions, Duties & Responsibilities:

Under the supervision of the Clinical Coordinator and direction of Primary Care Provider, responsible for daily operations in the medical clinic.

  • Uses and practices basic assessment and assists Clinic Supervisor as designated to complete initial assessment of new PACE enrollee.

  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.

  • Uses and practices basic assessment and assists Clinic Supervisor as designated to complete annual reassessment.

  • Performs basic nursing services as delegated by Clinic Supervisor.

  • Performs the administration of prescribed medications and treatments in accordance with nursing standards.

  • Maintains participants' medical record and fulfills Senior CommUnity Care charting and reporting requirements.

  • Observes, records, and reports participant's condition and reaction to drugs and treatments to physicians.

  • Triages participants to determine priority of care.

  • As delegated, assists Clinic Supervisor or Clinic Registered Nurse (R.N.) when necessary in signing off orders, ordering medications from pharmacy, and disseminating orders and instructions to appropriate personnel. Implements the orders written by primary care and nursing orders when necessary.

  • Instructs participants and family regarding medications and treatment instructions and care plan.

  • Provides education and training on care and treatment issues of the frail and elderly as requested.

  • Performs other duties as assigned.

Job Requirements:

Required Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Graduate of a school of professional nursing required. Current North Carolina Practical Nurses License (L.P.N.), Cardio Pulmonary Resuscitation (CPR) certification required.
  • A minimum of one year of experience in working with the frail or elderly population required.

Preferred Qualifications:
  • Intravenous Venous (IV) Certification preferred.

Knowledge, Skills and Abilities
  • Broad knowledge base of health care problems of the frail elderly.
  • Knowledge of medical equipment and instruments.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Skilled in identifying problems and recommending solutions.
  • Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
  • Skilled in developing and maintaining department quality assurance.
  • Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  • Ability to react calmly and effectively in emergency situations.
  • Knowledge of medications used in geriatrics and adverse reactions.
  • Skilled in wound care.
  • Pass background screening (drug, employment history, and criminal)
