NURSE HEALTH NAVIGATOR

RESPONSIBILITIES/REQUIREMENTS:

AN RN degree (BSN preferred) and 1 year of nursing experience required; psychiatric, community-based nursing experience strongly preferred. The position requires a RN license for the State of Pennsylvania and a Certified Drug and Alcohol Counselor certification is desired. Also requires a valid PA Driver's License and pre-employment screenings and clearances.

Work Schedule:

Monday through Friday, full-time, with flexibility and ability to work weekends and holidays when required.

ABOUT THE ROLE:

As a member of the Service Coordination Work Unit, this professional level Nurse Health Navigator position consists primarily of duties requiring advanced knowledge of health, mental health and substance use and its effects. The position is designed to exercise discretion and independent judgment in providing multi-disciplinary community-based services to the Service Coordination Unit which provides services to adults with severe mental illnesses and addictions. The Nurse Health Navigator is chiefly responsible for providing linguistically and culturally appropriate education on the importance of Wellness, with an emphasis on education to the unit and clients. The Nurse Health Navigator will track clients who meet the metrics set by CCBH for Wellness, will provide required documentation of those metrics, will coordinate appropriate care with the assigned Service Coordinator, will provide documentation of all referrals, follow-ups, and educational trainings for tracking purposes, and will screen persons served at regular intervals as needed or requested via phone or in person to determine clinical outcomes and engagement in treatment.

The duties of the position include the ability to interface with various members of the multidisciplinary team as needed, assessing the client from a holistic perspective, and acting as a liaison between disciplines, teams, departments and medical care for assigned clients. Major emphasis will be as liaison and agency representative with the BHHP initiative as coordinated through CCBHO. The Nurse Health Navigator must be able to act and respond appropriately and independently, using obtained experience and education in sometimes unpredictable situations, in coordinating and managing aspects of the client's behavioral and physical care and will champion their duties using Pittsburgh Mercy's philosophy, values, policies, procedures and standards.

In addition to maintaining departmental policies, procedures and goals, the above-stated job functions are to be performed in accordance with all applicable laws and regulations.
