JOB SUMMARY:

The MHRS-RN is directly responsible for conducting Mental Health Rehabilitation Service (MHRS) activities in accordance with the regulations of the Department of Behavioral Health and VOA Hope Center- DCs operation as a Mental Health Rehabilitation Service Core Service Agency. In this role, the MHRS Registered Nurse assists the Mental Health Providers in the coordinating their mental health care and management. The nurse will implement and evaluate patients plans of care for appropriateness to individual patient's needs, assist in the planning of group and individual health education classes for consumers of the CSA. Performs duties of assessment and clinical activities according to DBH regulations. Coordinates care management relationships with both internal and external health care providers to ensure healthcare collaboration on behalf of the consumers that we serve. Approximately 25% of the time, the RN will be engaged in activities out in the community and/or the consumers natural setting as needed.

REQUIREMENTS:

  • Successful candidates must be a graduate of an accredited school or program of nursing, BSN preferred.
  • Possess a current, valid, unrestricted Registered Nurse licensure by the District of Columbia Board of Nursing or must be willing to apply for DC licensure within 90 days of hire.
  • Experience working in a multi-cultural human services non-profit organization.
  • Ability to learn management systems (i.e. Paycom, Health Stream), creating learning management system curriculum as well as conducting and facilitating virtual and in-person trainings.
  • Experience driving and implementing evidence-based practices (EBPs) and aligning EBPs to diverse integrated service lines (i.e. Intellectual/Developmental Disabilities, Homeless Services, Justice-Involved, Behavioral Health, Mental Health, Substance Use Disorders, Primary Care, Veterans, Supportive Housing).
  • Experience with continuous quality improvement systems and development of quality plans for areas of residential and non-residential settings in the behavioral health and homeless services for veterans and family populations.
  • Experience with CARF, ACA and/or other accreditation bodies (i.e. JCAHO, NCQA).
  • Experience with diversity, equity and inclusion initiatives and implementation.
  • Experience working with and deploying electronic health records (i.e. Foothold/Awards).
  • Experience with and knowledge of diverse funding streams (i.e. fee-for-service, cost reimbursed, pay for performance, Medicaid billing, MCO contracts, etc.).
  • Ability to effectively collaborate and facilitate change, interfacing with all levels of the organization while managing multiple projects over multiple states.
  • Strong problem solving and execution skills.
  • Proven change management skills with excellent verbal and written communication skills.
  • Experience working with state health plans, MCOs, ACOs, HEDIS measures a plus.
  • Bilingual in any language a plus.

RESPONSIBILITIES

Majority of the tasks performed by an RN are regardless of their workplace environment, whereas some solely depend on the clinical settings. Here are some of the prominent RN duties and responsibilities:

  • Undertake various office responsibilities: answer clinical calls, reply to clinical messages and execute triage within the scope of the registered nursing practice in an accurate and precise way under the District of Columbias Nurse Practice Act.
  • Help to coordinate patient referrals and take part in the training and orientation of new employees, and interns as needed.
  • Call pharmacies for ordering prescriptions and store supplies as needed including other duties not listed.
  • Review appropriate paperwork for documentation of clinical services and clinical assessments.
  • Perform and aid in phlebotomy - analyze patients, look over the requisition form for requested tests, equipment needed, and patient and puncture site.
  • Enter the data related to the record of patients in computer systems.
  • Keep the health records updated.
  • Evaluates and reviews the work of medical providers for appropriate treatment and chart documentation under supervision of the Health Homes Director
  • Taking and documenting the patient's vital signs such as respiration, temperature, blood pressure, and heart rate.
  • Interview patients and examine their medical history.
  • Regulate and Administer Vaccinations and Injections
  • Educate and counsel patients to avoid unhealthy lifestyles and routines.
  • Inform the patients about the doctors order or test results via mail or call.
  • Assist Health Homes Nurse Care Managers and the Behavioral Health Clinic to develop best care plan for the patients.
  • Help the patients to understand documentation or paperwork.
  • Talk to the patients about their present problems, allergies, medications and keep an eye on their medical records.
  • Talk to the patients and their families about their treatment plan, tests, and diagnoses.
  • Taking verbal orders from the MD/ Psychiatric APRN.
  • Coordinate with pharmacies on behalf of consumer and provider.
  • Administer IM and SQ Medication at an appropriate time, as ordered by MD/ Psychiatric APRN.
  • Manage adequate inventory of examination room supplies and set the rooms.
  • Assess each consumer for bio-psychosocial needs, provide and coordinate referrals and make sure that the consumer receives the requested assistance.
  • Coordinate with Health Homes Director and Mental Health Providers to facilitate, track and monitor corrective action plans and systems from an organization wide lens.
  • Ensure compliance with state, federal and other regulatory standards.
  • Provides clinical direction to assigned CSW/CMs via ongoing individual and or group supervision.
  • Participates as a member and/or leads a care team depending upon the consumer panel characteristics.
  • Coordinates care management relationship with external health care providers, including pharmacies and other primary and specialty providers.
  • Receives, identifies and follows up treatment and medication alerts reported by HIT tools
  • Consults with CSA/health home team about identified health conditions of clients and provides education training on chronic disease states, health coaching, medications and healthy-living
  • Makes initial contact with hospitals regarding client admission, conducts a medication reconciliation with input from the clients primary care physician
  • Tracks all required assessment and screenings, including a health screening and metabolic screening, are complete for each client on their case load.
  • Leads development and implementation of an integrated person-centered care planning related to physical health care needs of each consumer.
  • Provide educational training on chronic disease states and treatment protocols as needed
  • Identifies and assist with the implementation of self-management protocols for use by consumers
  • Conducts at least every 180 days a medication review and reconciliation with each consumer.
  • Dispenses medications and administers IMs to consumers within the MHRS regulations as indicated by the MD.
  • Obtain vitals, regulate vaccinations, and injections for consumers.
  • Conduct health education groups as needed.
  • Reports changes in patients' medical or mental condition to the providing physician or PMHNP.
  • Provide Medication Management services to non HH consumers.
  • Attend meetings in the community on behalf of the consumer (St Elizabeth, nursing homes, etc.) when needed.
  • Conduct visits to the consumers job site, home, jail, hospital room, and other natural settings as assigned and when needed.
  • Attend training as required to stay abreast of District of Columbia Departments of Mental Health and Department of Behavioral Health standards.
  • Provide crisis intervention to consumers on a walk-in basis and as necessary.
  • Ensures that all documentation pertaining to consumer services is completed in compliance with agency policies and funding source requirements.
  • Participates in the continuing review of service and participation in the performance improvement program.
  • Ensures that each consumers individualized needs are met.
  • Perform other duties as assigned.

Quality Assurance and Compliance:
  • Expand and support organizational competence, credentialing, specializations and accreditation in relevant areas such as, but not to: Healthcare integration, Multicultural Competence/Care, Trauma Informed Care, etc.
  • Participates in MHRS accreditation processes (i.e. CARF and ACA).
  • Guide the development and implementation of standards supporting an integrated care delivery system; data management to support decision-making; and manage health
  • information quality and outcomes (i.e. electronic health record choice for organization).
  • Must be compliant with the design and monitoring of the organizations continuous quality improvement and assurance programs.
  • Ensures and demonstrates compliance with federal, state, local, and accreditation bodies regulations and standards, as well as VOACC policies and procedures at all times

  • Assurance of the delivery of first-class services for the consumers we serve and organization wide, documented with data.
  • Improvement or increased stability in the consumers level of functioning.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
  • Operating office equipment requiring continuous or repetitive hand/arm movements.
  • The ability to remain in a sitting position for extended periods of time
