About the job:

We are looking for an experienced Licensed Social Worker with a passion for geriatrics. Our Social Workers are key players in providing support and service to our residents and their families. Our careers offer opportunities to make a positive impact in the lives of seniors every day.

Essentials:

• LTC case management
• Discharge planning
• MDS assessments
• Care planning
• Providing resources, and referrals to residents/families/staff members
• Coordinates resident admissions
• Coordinates and leads transfer and discharge team
• Completes assessments of residents as necessary for Care Planning, MDS, RAPS, PPS and other required documents
• Develops and promotes the department through marketing efforts throughout the professional and social community

Required Qualifications:

• Bachelor's degree in Social Work or related degree in social sciences; Licensed in the state of Minnesota
• Minimum of one-year of Social Work experience, in a healthcare setting; knowledge of home health care and public health settings; familiarity with Federal and State regulations and requirements for home health care
• Twenty-one (21) years of age, meet the MVR organization standards, and have a valid state driver's license
• Ability to read, speak and write fluently in English

Preferred Qualifications:

• 1 year of experience with LTC
