We work with people at home, at work, and in the community, through residential, employment, crisis response, behavioral, recovery, children's services and day services.

Our Fort Wayne, Indiana Group Homes are recruiting for Licensed Practical Nurses.

RESPONSIBILITIES:

• Float between small home settings with adults and children with intellectual, physical, and behavior health needs.
• Make long-term relationships with individuals served and their family member's.
• Enjoy personal office space
• Provide support to people who are not sick, but need wellness coordination.
• Work at slower pace than a hospital or nursing home offers.
• Function as liaison and advocate for individuals when dealing with physicians, psychiatrists, and consultants.
• Provide health care training to Benchmark staff including medication training.
• Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
• Participate in regular pharmacy reviews with the consulting pharmacist and follow up as needed.

QUALIFICATIONS:

• LPN with current Indiana license
• Experience in community nursing
• Valid Driver's License
