Description:
Housing Stabilization Coordinator Medical Respite (Full Time)

REQUIREMENTS:
• Bachelor's Degree in Human Services OR High School Diploma and 4 years of related experience required.
• Must have exceptional computer skills using MS Office suite of products.
• Must be able to obtain ACT 33/34 and FBI clearances

SCHEDULE: Tues-Saturday 3pm-11pm (must be flexible due to program needs)
(This is a non-exempt, hourly position)

ABOUT THE ROLE
Provide direct administrative, social and programmatic services support for the individuals served within the Emergency Shelter. Administrative skilled services include compliance with all policies and procedures of the Emergency Shelter as well as reporting matrixes for compliance; operational directives to maintain a safe/healthy environment; facilitates building security; completes intakes and assessments and data entry into the HMIS system within 24 hours; accurately completes all required documentation. Programmatic and Social Services support include social services assessment of all new, as well as current individuals served. Responsible for utilizing appropriate information to collaborate with Staff to complete and/or update individual goal plans; provide necessary support for access to items to maintain daily living while an individual served is engaged at the Emergency Shelter; monitor/deploy de-escalation techniques to maintain a safe and healthy environment for all individuals served; assists individuals served with all necessary documentation for moving towards a housing first model of operations and support. Actively engage in a team-based model of service delivery, whereby the Housing Stabilization Coordinator will be proactive in seeking additional external and internal resources to meet the needs of the individual served.
