General Summary

The End Stage Renal Disease (ESRD) High Risk Care Manager is responsible for establishing, implementing, monitoring, and evaluating patient-centered care plans for ESRD patients enrolled in the Partners Integrated Care Management Program (iCMP). Utilizing a patient centered approach, the ESRD High Risk Care Manager directly interfaces with physicians (nephrologists, primary care providers and other specialists), health care teams, inpatient and outpatient dialysis units, access programs, renal transplant programs, patients, and their care givers in managing the patient's care. The ESRD High Risk Care Manager needs to possess sound clinical judgment, effective problem solving skills, critical thinking, excellent organizational and interpersonal skills, experience with team based care, and the ability to multitask. Must also have knowledge of available health services for ESRD patients. Direct experience in dialysis and/or kidney disease care management preferred.

This will be a hyrid role - a mix of remote work with travel to Dialysis sites in MA and NH. Office location will be Somerville.

  • Registered Nurse, Bachelor's Degree Required
  • Current Licensure in MA/NH, preferably both. Will compensate for 2nd licensure if needed.
  • Required: Minimum five years nursing experience.
  • Preferred: Minimum of two years acute care or dialysis clinic experience. Other experience may include post acute care settings or care coordination/case management experience with Renal Disease Patients.
