Position Summary: The Eligibility/Housing Specialist will work with individuals experiencing homelessness in Santa Clara County to provide eligibility determination, and enrollment assistance for benefits including Social Security Disability, Covered California, and Medi-Cal. The Eligibility/Housing Specialist will also train other employees and volunteers to help clients with benefit eligibility. The Eligibility/Housing Specialist will work with landlords to help individuals throughout our San Mateo and Santa Clara county shelters to successfully transition to permanent housing. The Eligibility/Housing Specialist will conduct outreach in the community to build relationships with landlords and provide housing search education in the form of 1:1 counseling, facilitating educational workshops, providing landlord/tenant mediation. The position requires candidate to work independently, and collaboratively as a team with the ultimate goal of increasing housing opportunities for the agency and placing program participants in housing that suits their needs. This position requires knowledge of San Mateo and Santa Clara County (and surrounding areas) housing market trends, fair housing, landlord/tenant rights, and a strong understanding of the leasing process.

Position Duties Include:

  • Engage directly with homeless clients in shelters to assess needs
  • Assist clients with enrollment for: Supplemental Security Income, Social Security Disability Insurance claims, GA/CalWorks, Food Stamp benefits, Medi-Cal, Covered California, and county health coverage programs
  • Assist clients with referrals for primary care, mental health, vision, and dental care
  • Train other employees and volunteers in how to assist clients with benefit eligibility
  • Follow up with employees, volunteers, and clients to ensure follow-through
  • Interface with medical teams and facilities to support disability claims
  • Complete training to become a Certified Enrollment Counselor and attend on-going state and federal health navigation trainings
  • Track outreach data, maintain case files, and submit monthly progress reports of contractual goals.    
  • Attend shelter sites weekly case management meetings to update staff with client's benefit status.
  • Professional experience in real estate and property management is strongly desired
  • Demonstrated ability working with individuals and families from diverse ethnic and socioeconomic backgrounds
  • Must be computer literate; internet savvy, experienced with Microsoft Office (Word, Excel, PowerPoint, Outlook)
  • Excellent written and oral communication skills; comfortable presenting in front of groups
  • Strong organizational, detail oriented, and time management skills.
Position Qualifications

  • Bachelor's degree preferred
  • Driver's license
  • Computer savvy and strong writing skills
  • Covered California certified a plus
  • Knowledge of the unemployment benefits system
  • Excellent interpersonal skills and ability to interact effectively with diverse client population
  • A strong foundation in the ideas and practices that promote diversity, equity, and inclusion; and a commitment to ongoing professional development in this area
  • Drive to seek solutions independently and contribute to team goals
  • Engaging public speaker
