Responsibilities

The Diabetes Educator is responsible for coordination of patient diabetes self-management education and support through clinical assessment, education and documentation throughout all phases of diabetes related care.

The Diabetes Educator is responsible for providing patient education with an understanding and awareness of age specific needs. Assesses the biological, emotional, developmental, psychological, nutritional and patient/family education needs in providing care to the patient and family. Functions as a member of the diabetes management team along with the patient, physician, other clinical colleagues, family and administrative staff.

Education responsibilities may include inpatient and outpatient setting and may occur in an individual or group environment. Serves as a diabetes resource internal and external to the organization. The Diabetes Educator is able to independently perform job functions. Participates in WellStar and community events (e.g. ADA walk, Diabetes Day, Health and Benefit Fairs, etc.). Provides presentations to WellStar departments, civic and community partners.

Qualifications

Required Minimum Education

  • Bachelor's degree or higher in Dietetics or Nursing from an accredited institution/program.
Required Minimum Certification
  • Current applicable licensure/registration required in the State of Georgia (e.g. RDn or RN). Current BLS from American Heart Association for Healthcare Providers.
  • Certification as a (CDCES)--Certified Diabetes Educator preferred.
  • If not currently certified, applicant must obtain certification within two (2) years of hire.
Required Minimum Experience
  • A minimum of three (3) years of progressive experience as an RD or RN.
  • Experience in treatment and education principles of patients with diabetes preferred.
  • Bilingual experience preferred.
  • Pediatrics experience preferred.
Required Minimum Skills
  • Professional communication skills. Computer skills. Must be physically capable of initiating CPR.
  • Travel requirements include local via personal automobile.
  • Independent and self directed. Excellent organizational and communication skills a must.
  • Computer experience with Epic, Word, Excel, PowerPoint and/or other databases as needed
