Under the supervision of the Department Director or manager, conducts preventive health screenings, patient assessment, and education in the community setting as coordinated under the system-wide community health and wellness outreach initiatives of the Community Health program. Position interacts with patients and clients of all ages, from infants to elderly.

1. Assists in the assessment of the quality and effectiveness of assigned screenings and educational programs.

2. Follows established protocols, policies and procedures for health screenings, patient assessment, education, documentation, referral and follow-up.

3. Consistently demonstrates established safety/infection control measures in accordance with the Maryland State COMAR regulations and standard precautions.

4. Provide supervision and oversight over student nurses assisting with community vaccination efforts.

5. Administers vaccination at community vaccination sites.

6. Works in collaboration with the mass vaccination clinical team (i.e., pharmacy, clinical vaccine leads, and nursing students).

7. Promotes an understanding of the cultural differences and age specific needs in the provision of educational programming through the incorporation of the principles of education and adult learning.

8. Collaborates with the Director of Community Health/Outreach to present programs that meet the needs of the community and objectives of the Community Health program.

9. Collaborates with providers and administrators including, but not limited to nurses, physicians, community health workers, peer recovery specialists, etc., to present programs and provide services that meet the needs of the community and objectives of the Community Health program.

10. Presents a clear and consistent message to community through participation in events and activities that promote community awareness of University of Maryland Medical System (including its affiliates and facilities) services.

11. Has a thorough understanding of established policies and procedures and clinical protocols for health screening, patient assessment and education, documentation, referral, and follow-up.

12. Conducts preventive health screenings, patient assessment, education, referral and follow-up (consistent qualifications and experience) in a variety of community settings.

13. Completes yearly competency testing.

14. Performs related work as assigned

Current Maryland license in good standing as a Licensed Practical Nurse (LPN). Basic Life Support - Health Care Provider (BLS-HCP). Two (2) years of professional experience required, including demonstration of healthcare-related competencies and skills. Experience in public health or occupational health preferred.

Associate degree in a medical, social or behavioral health field or graduate of an accredited school of practical nursing required OR Medical Assistant, phlebotomy certification.

Working knowledge of adult and child health education and learning theories generally acquired through formal education process and/or previous experience in community health; minimum of one (1) year clinical or health education teaching experience.

Successful completion of Community Health in-service training with a minimum score of 80% on required tests.

Demonstrated knowledge of health promotion concepts.
