Clinic hours: 7:00-5:00pm- flexible work schedules- some weekend health events may be required. No Nights required.

Clinic locations: Downtown Atlanta and Chamblee, Georgia.

POSITION SUMMARY: Under the supervision of the Clinical Nurse Manager, provides clinical direction of paraprofessional personnel in an outpatient clinic environment serving clientele consisting primarily of uninsured persons, homeless persons and/or immigrants. RN will oversee Vaccine For Children's Program, maintaining accurate inventory counts and ensuring compliance with the vaccine management protocols. Is responsible for conducting health screenings, education, and referrals for medical services. Health screenings include blood pressure, glucose, vision, urine, hemoglobin, TB screenings, and flu shots. Obtains medical history and evaluates the overall health status of the individual. Collaborates with Health Educator and also conducts one-on-one educational sessions on nutrition, diabetes, hypertension, vision care, tuberculosis, anemia, and other primary care health topics. Collaborates with Medical Referral Specialist and makes referrals to Mercy Care and other appropriate healthcare providers. Participates as a team member in the patient's plan of care. Provides support and acts as a resource for physicians and other healthcare providers as necessary. Complies with department policies and procedures, and supports the Mercy Philosophy. Participates in health fairs or community events on weekends as requested 3-4 times a year.

*Provides patient care and/ or support activities appropriate to ages served; primarily adults ages 18-65 or geriatric patients ages over 65. May also include care for infant age 0-1 year, child ages 1-12 years or adolescent ages 13-17 years.

EDUCATION REQUIREMENTS: Associate's Degree or Bachelor's Degree in Nursing. CPR/BLS certified.

EXPERIENCE REQUIREMENTS: At least two years related experience preferably in an outpatient care setting or outpatient education program. Desire to work with underserved and/or disadvantaged populations.

JOB KNOWLEDGE: Ability to effectively assess and evaluate patients, to make and implement plans. Knowledge of adult and adolescent healthcare, patient education, diabetes management principles, and home care continuum. Knowledge of growth and development including the adult and children, ability to adapt teaching to the age of the learner, including, age 18-65, geriatric over age 65, low literacy populations, and patients with special care needs. Excellent communication skills to develop and teach programs.
