We are looking to hire a Clinical Resource Manager for our Medical Center in Nampa, Idaho!

The Saint Alphonsus Medical Center in Nampa, located at the corner of I-84 and Garrity Boulevard, offers state-of-the-art, best-in-class healthcare to residents of Canyon County. This 100-bed hospital that spans more than 240,000 square feet, features a complete diagnostic center, 6-suite surgical operating theatre, pre/post-operative holding and recovery rooms, 10-bed short stay observation unit, spacious and private patient rooms, and an 18-bed intensive care unit. Built with preventive and ambulatory health in mind, the facility accommodates the latest information technology, updated diagnostic and treatment technology, and an environment proved to reduce patient stress and recovery times.

About this position: As a Clinical Resource Manager you will provide concurrent assistance and support to physicians and other clinical members of the health care team in coordinating the delivery of services for a select group of patients. You will also help achieve quality clinical and cost outcomes, incorporate real-time contacts with medical staff, hospital personnel, and outside organizations to establish specific treatment, cost, and transition targets and, facilitate transition planning.

  • Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. .
  • Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable).
  • Ensure the accuracy of documenting services and supplies provided to the patients.
  • Completes a screening/assessment of physician assigned cases to determine medical necessity/status determinations and transition needs.
  • Reassesses, monitors, and modifies as appropriate.
  • Uses established criteria to determine correct level of care for acute patients entering the facility.
  • Performs continued stay reviews as needed and requested by third party payers and Medicaid.
  • Responsible for appeals, denials, as appropriate. Utilizes physician advisor referral as appropriate.
  • Will adhere to department established process in reviewing 30 day re-admissions.
  • Follows established patient choice policy.
  • Documents assessment of primary and back up plans along with communications to patient , family and care team.
  • Participates in care coordination activities for those patients identified as requiring case management interventions to decrease fragmentation, increase communication and ensure effective implementation of a transition plan.
  • Works collaboratively with healthcare team members to efficiently allocate resources.
  • Documents and accurately completes data elements in electronic database.
  • Documents interventions taken to meet the needs of their individual patients in Power Chart.
  • Actively participates in department staff meetings and department sub teams.

The typical working schedule for this position is Monday - Friday.

General Requirements:
  • Colleague must have an RN license, as defined by their primary work state (Idaho or Oregon), issued prior to hire date. Those colleagues hired before January 1, 2020 must provide licensure or proof of application in process in their secondary state by August 28, 2020. Individuals hired in this role after January 1, 2020, must provide licensure or proof of application in their secondary state by August 28, 2020 or within six months of hire, whichever date is later.
  • BSN preferred
  • 3-5 years of varied hospital clinical experience required.
  • Experience in case management, home health, and/or insurance industry preferred.
