Job Summary:

As a member of the multidisciplinaryhealth care team, the Clinical Case Manager assesses, plans, implements,coordinates, monitors, and evaluates options and services to meet anindividual's health care needs through communication and available resources topromote quality cost-effective outcomes. The Clinical Case Manager facilitates access to all core medicalservices and coordinates comprehensive HIV primary care service deliveryplanning for patients through assessment, service facilitation, referral,advocacy, and follow-up. By working collaboratively with the patient and thehealth care team, and utilizing established Levels of Service Criteria, theClinical Case Manager plans and implements with the patient a Comprehensive Medicaland Health-related Services Care Coordination Plan, updates the plan as needed,documents all care and service contacts, monitors patient progress, andcontinually reassesses the individual's need for level of service. The ClinicalCase Manager provides treatment adherence readiness counseling and treatmenteducation and support, in collaboration with the other members of the healthcare team.

Possible exposure to hazardous material, hot and cold, blood borne pathogens.

-Be innovative Education & Experience Position Qualifications: Required:
-Master's degree in Counseling Psychology (MS), Master's degree in Social Work (MSW), Bachelor's degree in Nursing (BSN) or equivalent educational/experience combinations will be considered.
-Current RN licensure required in the state of OK by an accredited school of nursing.
-Two to five years professional experience in health and human services setting(s)

Skills, Proficiencies, and Knowledge:
Knowledge of and experience with local health andsocial services care coordination, including eligibility assistance necessary.Excellent interpersonal communication skills, and written planning anddocumentation skills required. Demonstrated competence in working withvulnerable populations required. Must be able to travel locally within the 918area of Oklahoma,have dependable transportation, and be sensitive to rural communities andcommunities of color, gay/lesbian/bisexual communities, and persons who havebeen or are substance using/abusing, or are incarcerated or homeless.

Preferred:
-Previous Case Management experience
-Experience in and knowledge of HIV direct care highly desirable
-For MS/MSW. LCSW or LPC licensure is preferred
