POSITION SUMMARY:
This is a predominately direct services position providing assertive outreach, assessment, community resource linkage, and care management for the target populations. Additional supervision or project coordination responsibilities may be included. Participates in health fairs or community events on weekends as requested 3-4 times a year.
1. Be a resource to both the medical and behavioral health clinic by linking individuals to community resources and other supports to promote community integration
2. Responsible for developing treatment plans, individual progress notes within 24 hours following session, and group progress within 48 hours following each session, for a collective minimum of 30 encounters each week.
3. Consult and collaborate with all team members including psychiatrist and Lead BH Clinician on a regular basis
4. With the individual's consent, engage family members, or other members of the individual's identified support system
5. Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual
6. Complete discharge documentation in a timely fashion
7. Participate in quality improvement and data gathering and reporting as assigned
8. Complete all required documentation in accordance with organizational policies and procedures
9. Assist with other grant programs and objectives that fall under the Child & Adolescent Behavioral Health umbrella and as assigned
10. Perform administrative duties including data reporting and collecting and health insurance payor registration and updates.
11. Complete paraprofessional training if applicable in accordance to the DBHDD Guidelines
EDUCATION REQUIREMENTS:
A Master's Degree in Social Work, Psychology, or Human Service Required. License eligible in mental health field preferred.
EXPERIENCE REQUIREMENTS:
Minimum of 3 years of related experience working as a mental health professional in the mental health/social services field.
JOB KNOWLEDGE:
Individual must be able to assertively build working relationships with this targeted population, provide ongoing comprehensive psychosocial/community living needs assessments (including evaluation of mental health and substance use disorders, with consultation), assist clients in prioritizing their needs, and facilitate linkage to needed community resources. These activities are to be conducted in community outreach settings (e.g., mobile medical clinics, psychiatric facilities, community kitchens, shelters, neighborhood sites) and through telephone intake. Must be able to make decisions both independently and with an interdisciplinary medical, educational and social services team. May assist with project coordination, to include reports, monitoring, etc. when called upon. She/he must possess and continue to develop working knowledge of community services and how to access resources, and be able to initiate and maintain cooperative working relationships with clients and those serving them. Must be able to recognize and describe symptoms of mental illness and substance abuse and make appropriate referrals for further mental health evaluation and treatment, including the communication of clinical observations to appropriate evaluating staff. Must be able to maintain appropriate clinical and personal boundaries and a positive, healthy role model for clients and colleagues.
