Scope of Position/Position Summary
This clinical support position serves the Suicide and Trauma Reduction InitiatiVE (STRIVE), one of the Department of Psychiatry & Behavioral Health's specialty care programs of distinction focused on suicide and trauma. The Clinical Case Manager facilitates a safe transition to efficient, effective, and compassionate next level of care that ensures patients and research participants are linked to medical and behavioral health services. The Clinical Case Manager monitors the patient care process and physiological needs as well as collects data to evaluate and improve the timeliness and appropriateness of care. The Clinical Case Manager collaborates with physicians, psychologists, social workers, nurses, research staff, and other health care workers to provide comprehensive case management related to treatment, follow-up evaluations, and communication of care plan. The Clinical Case Manager provides services to support the mental and physical health of patients and research participants independently and in collaboration with other members of the team. The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.
Function
Case Management organizes patient care activities to facilitate the appropriate delivery of health care services and transition of care across the continuum.
Sub-Function
Case Management collaborates with research and health care team members to develop a plan of care for each assigned patient throughout study and/or clinic enrollment. Ensures patient progresses towards desired outcomes by monitoring patient care through assessments and/or evaluations. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.
Minimum Qualifications:
Bachelor's Degree in Social Work, Public Health, Allied Health or other clinically related field. Licensed Social Worker, Licensed Independent Social Worker, Licensed Independent Social Worker - Supervisor, or other advanced license (ex: Respiratory Therapist, Occupational Therapist). 0 years of relevant experience required. 0-2 years of relevant experience preferred.
