Position Profile

Plans, provides and evaluates patient-centered care given within a team by effective utilization of staff members to meet patient/family needs. Supervises employees and participates in selection, orientation and evaluation. Contributes to the evaluation of quality care through the identification and resolution of problems.

Responsibilities

  • Promotes quality service delivery by serving as a resource to patients, caregivers, providers an staff to help facilitate and promote quality care.
  • Oversees and coordinates staff assignments to meet the clinical needs of AZSC patients. Maximizes the utilization/efficiency of the care delivery team.
  • Adjusts communication style based on target audience's fluency and understanding. Adjusts actions based upon the situation and the response of the patient and family.
  • Assesses patient/family needs.
  • Provides effective instruction regarding symptom management. Provides education related to disease process, self-care techniques, end of life care. Reviews medication use with patient/family and provides medication information, counseling and education.
  • Provides emotional support.
  • Works with and/or defers to interdisciplinary team to produce positive patient outcomes.
  • Contacts the attending physician for notification of change in condition and/or for obtaining advisement/orders.
  • Communicates and coordinates patient needs and care with other HOV departments and external customers in effort to effectively and efficiently meet patient's needs.
  • Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services.
  • Excellent verbal and written communication skills. Ability to communicate complex and sensitive information.
  • Excellent critical thinking/problem solving skills.
  • Ability to function independently and effectively anticipate needs and problems.
  • Ability to recognize capabilities and utilize others/resources to optimize patient care.
  • Ability to deal with confidential information in a professional manner.
  • Adheres to high standards of personal and professional conduct.
  • Promotes hospice referrals/business development.
  • Provides inspiring leadership, engendering loyalty and trust and leading by example.
  • Manages conflict effectively while practicing tact, flexibility and open-mindedness.
  • Hires, supports and retains high performing staff.
  • Empowers staff and delegates/shares responsibilities.
  • Maximizes HOV resources.
Minimum Qualifications
  • Associates degree in related field.
  • Bachelor's degree in Human Services or related field preferred.
  • Current state of Arizona license as a nurse. (If incumbent is a nurse.)
  • Must have and maintain a valid Arizona driver's license, maintain automobile insurance coverage and have access to an automobile.
Preferred Qualifications
  • Prior leadership experience in a healthcare environment is strongly preferred.
  • Previous palliative care, hospice or home health experience strongly preferred.
