Job Description Summary:
Community Health Center, Inc. (CHC) with offices in Connecticut, Colorado and California, is one of the country's most creative and dynamic providers of primary medical, dental, and behavioral health services, and a leader in practice-based research, health professionals training, and use of innovative technologies to advance health and healthcare. CHC is designated as a federally qualified health center and a patient-centered medical home by HRSA, the Joint Commission, and NCQA, respectively. We deliver more than 500,000 patient visits per year from primary care hubs and community clinics across the state of CT, all connected by technology and common standards for quality. We employ several hundred medical, dental, and behavioral health providers who are engaged in practice, teaching, and research. Our Weitzman Institute is devoted to research and practice transformation and is recognized around the country as one of the premier research institutes focused on improving health care and health outcomes for special and vulnerable populations. In addition, the organization has developed three wholly owned subsidiaries from the original pilot developments within the Weitzman Institute: the National Nurse Practitioner Residency and Fellowship Training Consortium (NNPRFTC), the National Institute for Medical Assistant Advancement (NIMAA), and ConferMed.

Job Description:

Healthy Communities is CHC's Public Health AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities also serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them. Service delivery will directly address the public health crisis in vulnerable communities and make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community based assistance. Additionally, AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services.

This is an AmeriCorps Member role - Mobile Health Program Assistant - that supports the Center for Key Populations in Middletown and surrounding areas.

Terms & Program Benefits:

  • One-year service term from September 1, 2022 thru August 31, 2023
  • Serve a minimum of 32 hours a week for a total of 1700 hours during service year
  • $16,502 stipend over the course of one year, paid bi-weekly
  • An education award of $6,495 at the end of successful service term completion

As a Mobile Health Program Assistant for the Community Health Center, Inc., you will:
  • Conduct patient education workshops, patient engagement activities and data collection in support of program's performance measures.
  • Work with the CT River Valley Farm Health Program (CRVFHP) to assess the need for Mobile Health Unit (MHU) clinics on a weekly basis.
  • Work with CRVFHP Program Manager to schedule, plan, implement and monitor weekly clinics including services like patient education, overdose prevention, clinical care and oral healthcare.
  • Work with CHCI operations team to develop and implement a registration process that works for the MHU including a process for walk-ins.
  • Work with the CHCI CKP team to integrate other programs into weekly and monthly MHU clinics.
  • Coordinate the overall schedule of the MHU organization wide to ensure that it is booked appropriately.
  • Work with MHU Program Manager to develop a curriculum for training for MHU staff, facilitate staff trainings and implement strategic training plan.
  • Develop community network of partners to assist in gathering resources and referrals for use by MHU staff.
  • Work with communications team to develop and implement a strategic outreach plan that specifically targets populations who will utilize the MHU.
  • Work with communication to develop and disseminate printed materials and social media information about MHU services on a regular basis.
  • Participate in Community Forums, advisory councils and other planning bodies to ensure that community service gaps are identified and addressed.
  • Work with CHC teams to identify gaps in services produced by Covid and address them through MHU services.
  • Ensure adequate access to Covid testing and vaccination for hard to reach populations.
  • Work to address other vaccination and preventive healthcare needed in target populations.

Qualifications:
  • High school or equivalent required
  • Proficiency in Microsoft office and internet-related applications
  • Excellent time management and organizational skills
  • Excellent oral and written skills
  • Successful clearance of all required criminal history checks (NSCHC)
  • Able to travel between CHC sites and in state

Preferred:
  • Associate's degree in public health, social work related field
  • Bilingual Spanish/English
  • Working knowledge of target population and resources available in the community.
  • Experience in providing crisis intervention, coordination of services, advocacy and community outreach in working with vulnerable populations
